Registration of Death
The Embassy is authorized to register a death of a Sri Lankan citizen, who passes away in the United Arab Emirates under Section 07 of the Consular Function Act.
The application for registration of death should be made within three months of such death.
Application for registration of a death may be made after three months, but within one year of such death, and in exceptional circumstances with a written submission by the NOK of the deceased, concerned party/parties reasoning for late registration.
REQUIRED DOCUMENTS FOR REGISTRATION OF A DEATH
- Duly filled Declaration of Death (Registration B 11 or Registration B 15 Form) as applicable by the informant.
- Original Death Certificate issued by the Health Authority of Abu Dhabi/Al Ain, UAE together with its English/Arabic translation attested by the Ministry of Foreign Affairs of UAE.
- Death Notification mentioning the cause of death of the deceased Sri Lankan.
- Original passport of the deceased after cancellation of residence visa in UAE.
- A copy of the Police Report, if applicable.
- A copy of the Birth certificate of the deceased.
- Any other documents as required by the Embassy.
